Are they all made of Death Metal? The Death Fork sure is
This being a C&V thread I presume we are neglecting 21st century happenings like the Trek hyperextending quick release lever

The one incident I have seen results like you have was a nearly Darwin Award level of user error. An old college friend of mine was trying to ride his LeMond while carrying half a dozen pieces of rebar. He wouldn't say why he thought this was a good idea. As he was riding, one of the rebar pieces slipped out of his hand and went into the spokes. The result was just as you pictured. He was lucky enough to get away with just a few nasty facial injuries. Hard lesson learned.

My favorite comment in that thread, and possibly my favorite comment ever on Bike Forums, was from jetboy: "dude, what a great WWE finishing move name: OOOOH NO!, Not the "Ishiwata Death Fork!" NO one should have to endure the pure humiliation!"

Were these prone to breaking? Genuine question, I wasn't aware.

The top tube of the '59 Schwinn Spitfire on which I delivered newspapers for a couple of years. My foot slipped on a wet pedal and I landed on that top tube and wished I was dead.

The Trek Ishiwata Death Fork is certainly real. It happened to me. But I'm still alive thanks to a minirack that held the thing together.
