The film and subject photo are actually sligthly older, being from 1942. The film may have been released in 1943 but was shot at last a year earlier, as the credits show a 1942 copyright..

Yes, that is a derailleur. While it was unusal to see a derailleur equipped bicycle in the USA during this era, they were not unknown. Given the era and North Road handlebars, the bicycle is most likley of British ancestry and the derailleur is probably a Cyclo.
