What is up with this stoker crankset?

A sorry-state 70s French tandem showed up in my habitual searching/perusing... If it were closer I'd be tempted to make a low-ball offer, just for the crankset. The frame color and details look a lot like my TH-8, though this one has braze-on bottle mounts and nicer dropouts. But what is going on with the timing chainwheel(s?) on this stoker crank?

It looks like it was set up for a kid’s crank on the stoker like the picture below. A third bottom bracket and a second timing chain are added so that the kid can pedal. The rust spot on the stoker seat tube is where the kid crank is mounted

Ah yes- I think you've got it! I was wondering what could've caused the big rust patch on the stoker seat-tube!
