Can you expand on this a little? I would think that if the cassette is simply tightened on the freehub body, it would be seated as well as it can be. How was it not seated properly and therefore "stuck out" too far to allow space between the small cog and the dropout? Was it actually too loose?

I think it wasn’t tightened down enough initially. In hindsight I rushed up the installation and didn’t want to over-tourque.
