Personally, I have no mixed feelings about your Casati, it is beautiful. I really like the adjustable derailleur stops on the head tube. Using a functional braze-on and making it even better. Can a conventional BB be used?
I have a fillet brazed Casati that I have mixed feelings about. I’m curious to hear the experts’ take on the welds here.
Seat post is held by an internal clamp. There’s a hex bolt accessible from the other side of the frame. I never have liked the inelegant transition from the oversized seat tube.

I suspect that there are threaded pieces brazed into the brake bridge and fork crown (like water bottle mounts) into which the pivot bolts thread.

What if you install them that way, but when they’re tightened, the caliper arms are pointing straight up? Or towards the side?

You could use spacers to position the bolt in the necessary orientation. It would take some trial and error, but should work. A nit-picker might measure the thickness of the necessary spacers and machine a custom one of the proper thickness for a clean look.
