What caused my DS cup to loosen on my BB?

Italian bike (Mondonico).
Phil BB.
10K miles.

I was having f der problems - not shifting into the large ring. I brought it into my LBS and he pointed out that the drive side cup had unthreaded about 1 cm. I had not noticed this as I tried to debug the problem.

I already have a crank puller, so I bought the phil cup nut for future problems. But I was wondering...

Is this a common problem for Italian threaded bikes? This has never happend to me in 35 years of cycling.

They have standard right hand threads, so they may come loose while riding

use locktite on the ds threads as it is an italian BB.

The LBS tightened up the cups and loctited them this time.

I bought just one phil tool because the bike wrench said I could get by with one. Did I need two? I could see where having two would be nice, but is it essential?

Yes. I mean the tools are expensive, unless you really are doing a lot of phil wood bb installs i'd let you LBS handle it (assuming they know what they're doing).
