It could be as you said the wedge catching on something but can't see what.
verify that the wedge or cone is knocked free of the stem. If you can hear it rattle around, terrific.
from time to time the expander distorts the steerer, that will result in the situation you describe. I would use Kroil.
in this case you might have to assemble a wood fixture to support the inverted frame/ fork by the crown on each side, whack the stem from the underside near the steerer.
when you are done, good chance the stem will show damage adjacent to the expander and be removed from service.

With that long assed stem, why didn't anyone suggest taking the adjuster cone off of the top of that fork and drop the fork enough to inspect the steerer tube? I have a feeling that it's either mushroomed as suggested by Kurt or the steerer is split below the threads.
