No picture but about 20 years ago the company I worked for bought a couple cheap Murray coaster brake bikes from WalMart for us to use to get around the plant. One day I hopped on one in a hurry and stood up to get rolling. The crank pin that indexed the chainwheel was welded on, not an integral part of the cranks, it snapped off and down I went onto the top bar. Luckily I have long legs so the impact wasn't too severe and my normal speaking voice returned after a only few minutes. About a year later I wiped out on the same bike when I hit a patch of hydraulic fluid from a leaking forklift. They scrapped the bike after that one and bought a 3 wheeler.

I bit it once while ghost riding a cursed Stella Arctic Cat like 6 miles. Whacked my leg good, and slightly tweaked one of the bikes.
That Stella was a basket case disaster, down to the stuck stem with 7mm stem bolts. Somebody else had a thread about one awhile back that was a pain, and I could totally relate.

The dust cap was rust-welded (I forget the term) in the left arm. First the hex rounded. So I drilled two holes to turn it, but it’s still not moving. I manage to remove the bb from the frame with the left arm still attatched. Now a hammer and punch on the cap — this thing will not budge. Now moving to sacrifice the arm for the bb. I hack two cuts through the bolt end of the arm and extract the remains of the cap. Got the bolt loosened. Now how to remove the arm from the spindle? I hammer at the loosened bolt — nope. So then I hammer at it with the spindle resting on on a piece of wood (lateral force) and it starts to come free.
Turns out the spindle has a couple pits in it.
PS: Though I wasn’t specific, I was initially thinking mechanical disasters. Note that below there are reports of personal injuries.

Bent bars, stem, fork, top tube and down tube. That's the result of going from 25MPH to 0MPH by not being able to avoid the three riders that went down in front of you. Of course, they walked away. I ended up in an ICU.
