The main thing I was thinking about is either botched modifications and repairs along with extreme abuse or neglect on a bike.

Then how is this a failed modification? I am surprised there's still a bike messenger in Chicago at this point, though.

Funny enough, soon after I saw this one of the suspension fork manufacturers (can't remeber which one) came out with a new model that looked a whole lot like this.
