+1 on all the advice about the Simplex drive train. If you end up not liking the aesthetics of the shifters, I know a guy who does…
While not Gipieme labeled, there were long cafe Simplex RDs that could handle larger sprockets.

My eyeball gauge says the hanger is twisted. The derailleur is unblemished so kind of odd.

The 1mm clearance is more than needed at that location, no running clearance for the chain is needed there.
Other brands of freewheels will usually increase this clearance.
Can you photograph the view showing the unexpected curve in the RH stays?
The Simplex derailers and well-oiled retrofriction levers can give friction shifting literally as good as it gets.
This bike deserves a Shimano Uniglide freewheel imho!
Some Ambrosio handlebars are found to have a 26.4mm center.

I believe you are correct about the frame being tweaked. The seat and chain stays on the DS aren’t straight. The stays on the NDS are arrow straight. Nice thing about this frame - I can take it back to the guy that built it and have him make things right. 🙂

Update - basically I have decided, mostly, to leave all of the bits on the bike. The mismatched wheels will be set aside and I have a sweet of wheels that will replace them. I have also cleaned and lubed everything so it is ready to go. I still need to get some KoolStops to replace the sintered Modolo pads which get many thumbs down.
The frame saga;
I was opening the rear to 130 mm to add gearage (7spd -> 9 spd) and I heard a sickening >CRACK<. WTF!!!???? The brake bridge had separated from the seat tube on the right. After thoughts of suicide I did the sensible thing. Message and picture sent to @gugie ("Paging Dr Gugie! HELLPPPPP!!!!) and he immediately called back. "How did that happen!?" "I dunno, started to spread the stays and...." His prognosis - bad braze many moons ago. As usual, he told me, "Don't worry. That's easy!" so I put away the arsenic I was going to chug. It was later discovered the right side chain stay lower bridge had separated too. Another bad braze, almost no silver in it. Nice catch @bulgie !

Yesterday I received an email that it was all ready to go so I swung by his place yesterday afternoon.
For now I'll coat with a color matched spray paint (a.k.a - half-a** it) and fully strip/refinish later.
PS - I was having my morning coffee and had an almost “eureka” moment. I grabbed the fork and…
I’m thinking of getting a decal made of the Fiestaware lady for a headbadge.
