8-speed Record shifting issues

So, I've done a complete restoration/overhaul on a mid 90's Colnago Master with 8-speed Record. Everything works great except for some minor issues with the rear derailleur that bugs me. Something like a third of the time, when shifting from the smallest cog to the next, and in particular, from the second smallest to the third cog, the chain won't shift, it just stays in a noisy middle position. It only happens when making single shifts, two or three gears at a time works perfectly. I've obviously tried putting a tad more tension on the cable but this only results in noise and overshifts, so it doesn't seem like cable tension is the issue?

The cassette, cabling, chain and pulleys are all brand new so shouldn't be a wear issue either.

Any ideas?

Have you checked that the hanger is straight?

So I’m guessing this is not Friction shifting but is Indexed.
Assuming chain length is correct, it starts to look like poor indexing.
Any chance you have a spacer in the cassette incorrectly sized?
or an incompatible brand?

Is the B-tension screw adjusted properly, proper chain length, shifter worn, cassette spacers in proper position (if they are not all the same)?

Ergos? Could be a frayed cable at the shifter, but I'm guessing you replaced all cables with the overhaul. Could be a broken spring carrier.

-I'm quite certain the chain is correctly sized (106 links in this case).
-I've tried all kinds of B-screw settings with the same result.
​​​​​-The cassette is a NIB Record 8-speed cassette. I've tried a different lightly used cassette before with the same result.
-The Ergos was serviced with all new G-springs and spring carriers.

I've spent a lot of time and money on trying to get everything "perfect" on this bike, so this problem, although minor, bugs me quite a bit. Almost leaning towards getting a brand new 12 speed Chorus groupset instead, but it would be nicer to keep the original components.

Is the shift cable routed on the correct side of the derailleur cable clamp bolt?

Did you use cable ferrules inside the Ergo handles? The newer Ergo handles specify to not use ferrules but the older ones may use them.
Did you cut all your cables squarely and use ferrules where they should be used?
Was the Ergo handle rebuilt properly? When I assembled my downtube Syncro shifters I put in one of the washers flipped the wrong way. It shifted fine for weeks, then completely failed to hold any gear suddenly on a ride. Apparently everything was tight when I put it all together but eventually some extra play in the assembly resulted in the ratcheting mechanism no longer working.

All cables were cut squarely, the inner housing rounded off with an awl and then filed smooth. I've used ferrules in the shifter body, but I'm sure that's how it's supposed to be with this generation.

I suppose I could dismantle the Ergo again and have a look.

Been a while since I've looked at my 8 speed. Am I remembering correctly that the splines on the cogs are all the same size and it's important to observe that the triangles on each sprocket are aligned? Am I also remembering that different same toothed cogs (e.g. 15,18, etc.) are necessary for different 1-8 positions?
