Bicycle helmet and flat repair kit come to mind as necessities. After that its whatever you want or need. Personally I like a rack of some kind (frame-mount or seatpost mount) with a bag to keep stuff in (flat kit, extra clothes, an extra bottle of water on longer rides, etc). I also like to carry a multi-tool; I find 2-3x per year its come in very handy to fix minor problems that could turn into big ones that would leave me stranded.

Most accessories are tougher to find these days because of COVID but if you are looking for something tough to find in normal stock circumstances I would suggest the following:
Spurcycle x Chris King limited edition bells (the regular ones are pretty easy to find) also the Spurcycle Tool could be added to that list.

Portland Design Works Bird Cages (out of production but easy to find at one point in time, they do have other cages now sparrows, owls, dogs, cats...)

Phil Wood x Blue Lug Rainbow Track Hubs

Cane Creek with their eeBrakes have done a number of special editions on them which are fantastic the latest is the "El TD" edition. (I have the El Chulo edition and they are fantastic looking)

There are also tons of other threads on useful accessories that aren't hard to find but you asked for hard to find and these should give you a little work not a ton but enough to make it hard.

I like those saddle bags that you can just twist off. I can't remember what they're called - not cheap, but very nice!

The Topeak Fixxer system is similar you can just slide the bags right off. The nice thing is you can buy the little F25 Fixers and put one on all your saddles and just swap bags from one bike to another quickly and easily.

Aside from what's mentioned above, I suggest either a small pump to carry on rides or a CO 2 cartridge and head (I carry both). I also always carry a Wolf Tooth Pack Wrench. The pack wrench can break and join chain links, act as a tire lever, a valve core remover and a stem lock nut wrench. It holds two quick links and is light weight and compact. There's a new model that includes bits to replace the need to carry an Allen Wrench multitool.

You may not use it very often, but when you need it, it's a life saver. I haven't used the new model but have used the original to save my ride and companion's rides.

I put a totally badass rider on every one of my bikes. Hardly anyone else has one. The price is pretty steep, but worth it.

You gotta have a bar buddy and a gremlin bell. These two items are typically gifted not bought.

A nice bright headlight and taillight. If riding at night, spoke lights are a good idea. An underseat bag to carry a patch kit, spare tube, tools, etc. to get you home in case of a breakdown. Don't forget a frame pump. Depending on where you ride, a bell might be needed. Nice to have, but not necessarily needed, is a phone mount. Also nice to have but not needed is a bike camera. I just got one for Christmas and mounted it on my handlebars. In case of reckless drivers making close passes, or if someone runs over me and drives off, hopefully I'll have it on video along with their license plate.

There is a thread hereabouts on snazzy valve caps, à la Syrene's post.
Not really a bike accessory, but I have seen some nice videos of bikes, taken by drones in 'follow me' mode.

A seat with a hole in the middle so you will never have to stop for bathroom breaks. At least that’s what the guy at the LBS said.

Only thing missing from Glen’s excellent kit is a folded one dollar bill for a boot in case of a severe tube cut. Otherwise, good visual.
