Why are handlebar extensions so thin; 22.2

None of my accessories are anywhere near that thin in diameter ( computer, cell phone holder, bell, etc.) What do you guys use for shims?

22.2mm is the standard size for flat bars. Perhaps your accessories were designed to clamp onto drop bars, which are 23.8mm. Regardless, some people like to use an old inner tube as a shim. It's grippy and easy to work with.

Shims can be purchased from better bike shops or you can make shims from soda can aluminium.
