discogram or diagnostic nerve block? That is the latest issue. Neurosurgeon suggested discogram, the guy who I went to see suggested nerve block but didn't want to step on any toes. He called the discogram old school. Thoughts?
As if my conundrum couldn't get worse, my insurance company and the hospital group that has an effective monopoly in the county are in a contract dispute. I can't see any of my doctors unless I pay out of network rates. The contract expired October 3rd and no solution. SMH

I have no experience with discograms, but it makes theoretical sense and certainly goes right to the disc issue, the idea being to reproduce the pain, along with providing some additional imaging. The nerve block is less of a deal and also makes sense, but is somewhat less specific, since multiple things other than disc pathology will feel better after a successful block. When I had my disc blowout, there was never any question about what was causing my leg to feel like it was having a ******g baby, but the transforaminal injections I got helped immediately and specifically. Bottom line, either or both would probably be helpful and both are pretty safe.
I really hope there’s an answer soon, but a as miserable as this all is, it’s better than adding to your troubles with unnecessary surgery, which has a non-zero chance of causing new problems even in the best of hands. The neurosurgeon who did such a heroic job on my neck and saved my spinal cord talked me out of lumbar surgery, even though he had a clear target, and got me through with injections. That was in 2013 and my left quad is still visibly atrophied.
That also sucks unbelievably about the third party contract dispute.

As a neurologist, a patient with serious cervical and lumbar disease, and a consumer of neurosurgery, my strong belief about interventions for lower spine pain issues is that the anatomy really has to map onto the symptoms before taking any action. In fairness to your surgeon, he has held off and is looking for proof before intervening, so thats good.
