I have been using sram for 5 years. It took a short ride to get used to them. One click is up, two clicks is down. I like as well as Shignamo(sp). Works good. you can use brakes while shifting.

Try tilting handlebar hoods up which will shorten your reach and tilt seat back. Even a very small adjustment will make a difference. Experiment but I think you have too much pressure on your arms.
