Might not work for you, but I found rehab, and then gym work, directed at the deep neck muscles after my surgical adventure, very helpful. I have mechanically limited extension, but still manage about a 4" seat-bars drop on the road bike and could probably go down a 5mm spacer.
I may also have gotten better at lifting my eyebrows and rolling my eyeballs up.
