I checked and chain is measuring out 0.75% elongated. I used to run them to 1% but I am guessing this may be the cause of the noise.

The Shiv is a TT bike, so does it have lever shifters rather than combined brake levers/shifters?

That could explain much of it. The clunk is likely the index stop on the shifter.
