It's OEM packaging. It doesn't have the guide pin because the actual pin is already started into the plate. This is to speed assembly at bike manufacturers. Just be careful while installing it....

Don't take the wheel in, take the bike in. Chances are the wheel has just shifted in the dropout or the brake calliper has shifted. It is extremely unlikely that you bent the axle.
