If "most riders" is the criterion, then even he weakest bike frame is safe from failure. Most bikes get ridden a handful of miles per year.

I don't think one can say with confidence that fatigue...

Fixed it.

There's no way to be sure why some random carbon frame failed. And given that carbon fiber has essentially infinite fatigue life, the reason is probably not fatigue.
