I grease the tapers and the bolt threads/washer and use the Campy tool uncheated. Been doing that since about 1978. Never had any come loose.

Works fine for me.

I ride all three. I can't tell the difference. The 5mm is less than a 1/4". I also ride several types of shoes (one even has insoles) and cleates and I'd bet that there is more than a 1/4" difference...
