These seem to help me. I don't have the exact thumb issue but enjoy changing position on a long ride for a bit of a rest. (A boarding bail led to some bone injuries in each hand.)

Constant pressure can result in nerve damage. To prevent this I add an extra layer of wrap on the top flat section of the handlebars and move my hands from there to the top of the brake levers when riding. Also bike gloves with gel padding may help.

Yeah, I've been riding my bikes that have down tube friction shifters. That shifting is easier on the thumbs.
