In other news of people not getting it, my dad's printer just died and he asked me about options. I recommended one and offered to just order it for him through Amazon, what with him being unemployed due to the casino shutting down. He declined at first, and wanted to go see what Staples had in stock. When I told him he shouldn't be going out unless he absolutely has to, he said he couldn't just stay inside and had to go out to a few other places anyway so "it couldn't really hurt". This is a man over 70, overweight, who spent over a decade working on a smoky casino floor. And I'm quite sure small-town CT doesn't have people wearing masks regularly.
He finally agreed to let me order it for him, but damn. I am really afraid of the butcher's bill on my family alone.

Agree. Most people live by their biases and ****ty intuitions and failure to understand probability is almost universal. Daniel Kahneman got a Nobel prize for showing what hard wired dopes we are. Stats is a way of and seeing the world as it actually is and those who learn and practice it come out way ahead. I suck at the mechanics, but I do understand the principles.

My decisions almost always boil down to continuing with drug A (which obviously isn't doing enough) or trying drug B (of which I, at the time, know almost nothing).

I don’t know. I am not very well informed, but I have heard arguments and experience for and against. It’s also it’s easy for me to say, working in an economically unconstrained setting. All I know is that the perverse incentive to over-test and over-treat has to be removed. Another related beneficial effect of capitation-based systems is price transparency.
Bundles are nice because physicians get rewarded for not doing that... the lower the overall cost while maintaining the same level of quality, the more the doc makes. it’s really nice in areas like Ortho where the surgeon/physician has a limited time treating the patient.
I don’t know that we’re ever going to have price transparency, billing has become way too complicated. Have you seen a hospital charge master lately? The average Joe will never get it.
