What can cause Peroneal Tendonitis of the ankle?

I have been suffering from ankle tendinitis for some time. what can cause this type of tendonitis in the bike setup?i saddle too low or high? cleats too far back or forward?

Holding your feet in the wrong position on the pedals. If you use cleats then you might need shims. Or just work at holding your feet in a better position inside your shoe.

I have always pedaled with a yellow bike fit cleat between the cleat and pedal (with the shim on the inside) without problems, the last change I made was to have moved the cleat back, perhaps this was what triggered the inflammation, I also use red cleats keo that we leave plenty of margin to compensate for any problems.

It might be something as easy to fix as cleat angle.
