Apologies for dragging up this thread, but I was busy when this discussion happened and missed it the initial time.
The original shared lane marking concept originated by James Mackay in Denver did used an outline arrow with a bicycle symbol in it, which looked like... a bike in a house.
Subsequent research and testing resulted in the design that was adopted in the US MUTCD. See Part 9 of the MUTCD for more information.
One of the research scenarios was to use a standard bike lane marking in a general-purpose travel lane. An interesting finding was that over half the test subjects said they interpreted it as making the entire lane a preferential bike lane.
