When I was car-free (8 years or so) I had to get to around after my rear D hanger got ripped apart. I was starting my business (machining) and made a super robust "thing" like that. It was super rigid, but every few miles the chain would come off. There is way more "flex" in the driveline of a bicycle than you would think. I installed a torsion spring, and presto, fixed!
Edit- Damn I was strong, I rode 7 days a week, rain or shine.

Would have made a lot more sense to shorten the chain, but it wouldn’t look as cool.

Derailleur? Not with that wheel. It's just a clever tensioner, and I don't see any need for it to be sprung. A shorter chain would be the more orthodox approach, assuming there's a single chainring, and assuming it's not a flip-flop with two different sized cogs (impossible to tell from the pic). This tensioner would adjust for such changes, simply by rotating it where it pivots on the axle.

See the setup IAB posted? Do you see the stirrup beneath the jockey wheel? That is to retain the chain because is IS going to go both up and down with enough travel to disengage. Just having that dessert fork that Yuri Gellar can bend with only his mind WILL NOT WORK.
