Why do hand cycles have friction shifting?
Front chainrings have friction shifting and indexed rapidfire for the cassette in front. Why not indexed all around?

Depends on how they are set up. I've seen some where you manually adjust the "front" derailleur at the hand cranks (lever attached to the derailleur), some that use friction shifters, some with frame mounted trigger shifting and some with crank mounted shifters. Personally, I like having chainring shifting on the cranks, but that is not a common set-up.

This was the standad for nicer road and mtbs from 87-95,most major groups groups did not bikes did not get full indexing tell the mid 90's''

Most new bikes from big box store and up have front and rear indexed shifting as standard.now.
You can convert the front to indexed by upgrading the shifter. My 1989 KHS had a non-indexed thumb shifter in the front (originally had a full Shimano "Mountain" Exage group) I upgraded to 3x7 SRAM 3 trigger shifters (also upgraded to a SRAM X-9 rear derailleur. I detest Shimano.) With the stock Exage front derailleur I have indexed shifting in the front now.
