If you're asking about a modern threadless stem there is no part of the stem inside the steer tube. A quill stem goes inside the steer tube. A threadless stem clamps around the steer tube and can be positioned up or down the steer tube depending on how much steer tube remains. If the steer tube is cut off with the stem at the lowest position, then that's it. No adjustment left.

I know the cap wasn't bottoming out and I fought it for months and finally moved the stem down to allow one 5mm spacer on top. It hasn't loosened since that change.

Addendum: I ordered a 13/14 mm just like the above from Amazon, to arrive tomorrow. I thought maybe I'd buy a 13/15, but I couldn't gt that tomorrow. So I went with the 13/14. Then I checked the rear wheel, and the cone and locknut there are BOTH 14mm. So I guess it's good I got the13/14.

Roscoe's heart condition has not worsened so the vet thinks he might be having kidney failure. He took some blood tests and we'll know more tomorrow. He seems fine today even though he was displaced today with his appointment and 4 house showings. One of them has already said they want to come back.
...and the awesome shovel continues to be awesome. Stored in an undisclosed secure location in the Northwest Metro Atlanta area.

If by shuddering you mean like a mechanical vibration that sounds like a transmission or final drive issue. Maybe even a motor mount. Even a timing chain. Check for a vacuum leak first to try and get the idle squared away.

I typically use regular ol' end wrenches for locknuts.

They had a new girl holding down the fort at the fish and chips joint today. She went just a little hard on the cod and a fair bit soft on the chips.
