Back when I started riding, I thought Sidi had a reputation for making crazily narrow shoes. Some time between then and now they started making their 'mega' width wide shoes and I think they may have overcompensated. When I first ordered a pair of Sidi shoes I assumed I would need the mega width, but when I got them I had to send them back because they were too wide. They make Mega width shoes in some of their nicer models, too, so you have some slection. THe other nice thing about their better shoes is that the 'treads' on the sole are replaceable so the shoes can potentially last longer than shoes from other manufacturers.
I have no experience with the other manufacturers of wide shoes, but they all seem to have a good reputation so are also worth a look.

I've got a EE foot and started with some Shimanos, which tend to run wide. I'm much happier with Sidi Mega shoes.

I've got a pair of the Lake cx165 wides that I love. Easy on the wallet too.

My Sidi Genius 5 Megas just arrived on my doorstep today. Wow, where have you been all my life...for years I've used Lake, Pearl Izumi, Northwave, etc. and always had a problem with foot numbness and pain.
Always held off on Sidis because of the price, but this time I said 'screw it' and bought a pair. Best decision I've ever made.
