Pulled out of his pedal. Not enough tension, or broke a cleat or a pedal. It happens. You almost always lose control and fall in some manner if you come out while sprinting. You yank the bars up on one side, expect to pull the bike straight while pulling up with the other leg, and suddenly that leg is in your chest, and the bike is still leaning the other way. Yeah, you're gonna fall.
It's a repost, but it's still cool to watch. His reaction is that of a true racer.

Ah okay. Never broken a pedal, or unclipped unintentionally on my looks. My mountain bike w/ SPDs on the other hand.. Speaking of which, I have trouble unclipping outwards on the looks. I can do it easily if I move my heel inwards, but then I hit the seatstay. Just sayin.

I saw this happen in person and my guess was either the cleat broke or just flat pulled out of the pedal. It's a 150 meter sprint up hill. Still got the win though! Too bad he was hauled off in an ambulance once the actual injuries settled in.

Sure it was. Even the rider had his wits about him to look at the finish line and know what was really important at that point.

Sure. The speed isn't the only determining factor. I know someone who crashed while descending a twisty mountain. He didn't withstand much... broken collarbone, broken rib, punctured lung... but his bike got a scratched brifter and a worn down aerobar.

It's clear the immediate reaction was adrenaline-fueled. In my (unfortunate) experience with bike and skateboard wipeouts that involved broken bones, there's an extra surge of adrenaline that kicks in right after impact that brings on a brief period of intense lucidity. This assumes of course you are conscious. Shortly thereafter, the color drains from the face, the knees get weak, and the pain sets in.
