Clipped out during a sprint today
Riding down a kinda busy 2 lane rd, and notice a motorcycle rider is about to turn right onto my road.
A few hundred feet later he's coming up behind me, I turn around, wave him over and ramp up the speed to 30 mph. He gets the hint and lets me draft him.
We're riding along at probably a steady 35 mph. He starts to accelerate slowly, so I sprint to keep his wheel.
After a few good kicks my right foot flies out of the pedal. I think I balcked out briefly, because the next thing I know I'm sitting on my top tube in a very unnatural position with my foot draggin across the pavement.
As if that wasn't bad enough my bike is getting really shaky and hard to handle because of the speed and my position. I manage to save myself and the bike, but my shoe looks like it met a belt sander; the toe box is almost gone.
The rider pulled over and asked me what happened and said he rekoned I was going 37-38 mph, because by the time he realized what happened he was doing 40.
New shoe time.

you're lucky.
one time when I was a mere lad I had a crank arm break off while standing during maximum power output at high speed and I stuck to the bedsheets for a month after.

Holy crap. Once, when I foolishly sprinted on SPDs, I ended up with a broken collarbone and no spleen for my trouble.

2009 Look Keo Carbon CrMo's with the tension 3/4 of the way up. The cleat has some scuffs on it, but otherwise all of the wear-indicators are as deep as they were when they came out of the package.
I guess it's my mistake; I never tightened up the tension all of the way on these like my others.

it happens with any pedal system. cleats get worn, dirt gets in there, springs wear in a bit, etc.
glad you didn't go down. I've collected a small number of scars and sore nuts over the years, but at least I can say that when I sprint now, I have a firm hold on the bars!

A teammate of mine did that on the San Diego Velodrome... right after they resurfaced it with a sandpaper like finish (for tire traction). That surface tor his skinsuit (and his skin) to ribbons as I swerved to avoid running over his neck, and he stood up holding his junk in what little fabric he could find. Glad you came out of it better than he did. Time pedals, fwiw.

I have done this a couple of times, but usually on the early end of a sprint so it wasn't too gruesome. I have found it to be more common when hitting it hard in a climb (lower rpm's/larger pedal forces). Once I pulled my foot out of my shoe that was still attached to the pedal. That sucked.
When I was in 5th grade I had a chain snap in a full out sprint. I was unconscious for over 20 minutes. (When I was in 5th grade (1965) there were no helmets or helmet police.)
