Has anyone combined a crank shortener and pedal extender?
I use pedal extender on left side to avoid heel strike.
I am considering adding a crank shortener also on the left side, and wondering if there are known issues with combining those together?

Would there be a need to? Maybe you found a nicer, high end system but the basic crank shorteners are thick enough the pedal spindle won't stick out the other side and gouge the crankarm, probably about as thick as your current extender. Rather than cludge together something like this would it not be better to buy a shorter crank with a wider Q-factor if something new or just buy a wider BB with a shorter crank if older square taper?

It's not an older square taper. The issue is that prosthetists point the toe out a few degrees on the foot on artificial legs and this is required to make the amputee walk efficiently. Also, I have a very non-textbook dynamic alignment to make me walk well. The ankle does not rotate, so that toe is always pointed out.
I have tried having a biking leg just for cycling, and that was a fun experiment for about 5 years, but not sustainable. Now I'm going to modify the bike to me. I'll take a look at shorter crank on 1 side with wider Q-factor. The crank shortener and pedal extender are only needed on the prosthetic side.
