Cyclists who swerve to the left to make a lane change and don't even bother to look back to see if it's safe to do so. Cyclists who pull out of sidestreets without even looking if there is a car or another cyclist coming. And salmon riders make my blood boil.

Drivers who pull into full intersections during rush-hour, then the light changes and they're stuck there blocking traffic, but they still have the audacity to honk and yell at me for cutting between them when I'm the one with the green light and they can't go anywhere.

What grinds my gears?.... actually they're not ground at all but are in fact cut on a CNC router table.

..... is when the bike lane is completely blocked by several delivery/personal vehicles, and having a motorist aggressively tailgate me the whole time I'm in the lane.

+ 1 zillion! Or the 'scathingly silent glare'.

Salmons, definately.

Full-kit cyclists in tuck position blasting past on the decends only to be passed on the climbs even though I've got loaded panniers.

Honkers, yellers, one-fingered saluters, buzzers, harassers in general that I never seem to be able to catch at the next light or stop sign. And I always forget to get their license number.

Here in Georgia, trucks are offered with turn signals optional. 99% of the truck owners on the road here bought them without turn signals. Yes, I'm with the OP.

cars pulled halfway into the intersection, waiting to turn right on red. Are you looking right at me, or are you looking right through me?

Getting a $150 ticket for quickly parking my car to help a person collapsed in the street 100 feet back because my rear left tire was in the bike lane, but I'm constantly having to ride around delivery trucks that are not only parked in the bike lanes but also sticking out into the traffic lanes to the point traffic backs up over a block.

Don't get me wrong...I'm all for enforcing the bike lane, but come on...really?

I don't think I've ever seen a city bus (here, or back in Ohio) use a turn signal.

Cops who drive right down the middle of a bike path(not lane, path) without any indication of what they are doing. Had to pull into the grass, but I probably should have just taken it and been run over.

... parents who don't teach their kid bicycle etiquette. Stick to the right, don't zig zag all over the MUP, ride single file, don't block entrances with bikes while waiting for others, if you've gonna have a water bottle squirting contest don't cry like a big baby if you get squirted too.

Snarky commuters who don't understand the difference between a 5 mile stroll to work and 60 mile training ride, and therefore feel superior to the roadie who's more concerned with the 12 percenter he/she's got at mile 40. +1 to everything else you said, though.

Also: motorized cruisers, trucks, and folks who pedal with their heels. Oh, and sidewalk riders...my word, claim your effing lane.

Breaking a strand in my right shift cable. My gears ground shifting to smaller cogs until I gave a tug on the cable because the loose end was hanging up in the housing.

Red lights at the bottom of a hill.

This is one I get all the time: An intersection where the crossing street has a stop sign. A car at that intersection, with plenty of time to stop, cross and be clear of the intersection long before I get there. But they stop, then do the little "should I stay or should I go" dance, until I'm forced to slow way down. THEN they wave me through. Gee, thanks. The worst part is, I can't be mad at them, because they're TRYING to be polite. The irony is, if they'd been a**holes, it would have been better for both of us. I weigh 285 pounds - like the fighter pilots say, speed is life. Every time I have to scrub my momentum for no good reason, I cry a little inside (and a kitten dies.)
