My unprofessional opinion is you have little to fear about a STEMI but you could have a valve problem.
"Tapping" sensation could be irregular or incomplete valve closure, "pounding" to compensate for the leakage and maintain cardiac output... once again my very unprofessional opinion.
Dehydration and sudden changes in exercise intensity can exacerbate valve problems as heart input blood can be less than or more than expected (cardiac preload).
Stopping for a poop or a chain drop during a hard effort fits.
Be well.

Treadmill test with sensors on the chest, and a breather over the mouth, blood tests x2, urine test. I've also almost argued with the doctor as to why it wasn't a heart issue (according to them, I would have something in my blood that showed heart damage)
Thanks for the info, what exactly do I have to be concerned about with a valve issue?

Values issues, or other small "holes in the heart", can be essentially harmless, they just reduce efficiency. Really bad valve problems will strain the heart and possibly cause other problems by throwing off little bits of infection (vegetations / endocarditis) or clotted blood that stagnated around a faulty valve in an eddy. Essentially all is treatable, antibiotics, blood thinners, or worst case valve replacement or heart transplant.
Once again I am an idiot you should not listen to.

Fiesta time trial bike this AM. 3 laps with 3 z6 (>120% efforts) each lap and all other times 75%. Then one lap for good measure at 75%.
Most of SoCal and the west is having a heat wave but the coast was pretty nice with temps in the upper 70s.

I got 3rd at Tuesday Night Worlds, that's getting printed and put up on my refrigerator door!
Since it was over 100 we only had 30 show, and no real 'hitters'. We usually have between 50-80. We did have an outside team show up and those goons instead of accepting they were dropped, stopped waited at the top of the hill to pull their guy the last lap; didn't work though. Probably not the smartest thing to do but I spoke to 2 guys on my ride who are doctors (lol half my group rides are in the medical industry but no sadly dentists) and both said I was 'fine'.
I don't think its my best result, I did get 5th 2 years ago in the first group,

We KOM'd our Tuesday Worlds route this week. At first I shared the KOM, then a few more uploaded and I lost out by a few seconds.

Did Tuesday Wednesday Worlds. Raced hard and stupid. Wanted to see how deep I can go and what I have at the end, also get that mentality of going deep in to red back. Doing zwift races is just not the same. Still have some questions left in my head.

getting into short power build. with the new trainerroad workout classification, I did a level 4.0 vo2 workout today, a series of 4.5 sets with 20sec at 110% and 15sec @ 88%. Not bad at all, adaptive training isn't going to throw the really demoralizing vo2 stuff at me right away with this adaptive training stuff.

6x2' VO2max intervals. Felt really great. I had to get out early as it's going to be 108F here later today. Got out to the golf course loop just after sunrise and just drilled it. No better feeling than hitting all your wattage targets.

I've been building back some volume. Weeknight worlds last night. Went great for the team, still learning and trying stuff.
We worked well enough that nobody was going to succeed against us except for any of the cough cough Cat 1/2 talent. We're cannon fodder for one or two of those dudes.
I put in an early uphill dig to see who came, but wasn't paying attention and the uphill group speed was too high. So too many people were coming so abandoned that. This meant when the 1/2 guy went about 2 miles later, I couldn't pull through but twice for him when I went with him. If I hadn't dug earlier I probably could have rotated with him to finish.
Pretty tranquilo from there till the last attack where he got away. I'm still not great knowing "who's who" so mis-identified the jersey he had on as another rider. So, was thinking "let's let this sucker bake in the sun for a few miles". Much to my chagrin, the group expressed to me with only a mile to go "dude, you have to reel him in". Too late. Didn't work as a group enough before that to reel him in. We could have easily.
I was first loser in the bunch sprint second to the 1/2 guy who got away.
Was fun, great teachers around me. Always learning from those guys who've done that ride for years and years.

Hour of power, road bike, on Fiesta with 40 second >120% efforts every lap. Fiesta is ideally suited for this workout since for all intents and purposes, it is just a long track with 4.1 mile laps. I like the idea of making every pedal stroke count for 60 minutes - very Zen like. Beautiful day with more headwind on the back straight and a head/cross on the fast section past the dog park.
For the locals, my wife did 15 miles of climbing on Cardeno on Mount Soledad. Ugh.

Finished my biggest block ever @ 4,800 TSS over the last 5 weeks. I take an easier week every two weeks since I'm old, so it was 4 hard weeks and one slightly easier week. We spent the pandemic building up my ability to absorb more volume/intensity of training to try to get that next jump. The big thing from me was how I felt today. Yesterday was 4 hours with 2 hours endurance followed by 2x20 @ FTP with a 10 min endurance in between and then 45 min of tempo. Today, I went out and did a kind of race sim of 60 miles to see how it felt and it felt surprisingly good. Usually when I get fatigued, my leg speed drops hard, and it didn't today.I was able to put in a 21-mph effort with 4200 feet of climbing with great leg speed. This training stuff works.

Saturday - 2x20 km TTT with wife. She pulled the first one and I pulled the second one. Legs felt good.
Sunday - 2 hour endurance ride along the coast most high z2 low z3 with some sweet spot. Legs felt amazing. Recalibrated the power meter a couple of times.
If anyone was watching the Golf US Open at Torrey Pines that is my home course for golf and we ride along the road next to the golf course. The weather both days started off with a marine layer and then turned nice.

Double Z race morning, just finished the first one, have about 10 total min of cooldown before the second.
12 laps crit city A race - 3rd/9 starters, although more like second...
Some dude rode OTF the whole race, but he had some bot or something, Very erratic power and it would never go over 6.1 w/kg, although it was often there. It was quite surgey with z6 power most laps going over the rolling hills, then mostly settling in between. A couple riders got dropped and then it was 6 of us for most of the race. Some guy made an awesome move after the surgey hills and held it, nice work. I was the best of the rest apparently and did my usual sprint between 2-3 corners to go. 5-6 seconds of hesitation was all it took for me to comfortably coast in after the last corner. About 35 min at 4.2 average.
Up next... 5 lap/20 miles London Classique race, mass start all categories. edit: 28th/112, got dropped from the lead group
I almost thought I would miss the selection on the Lead In up the 5% grade where it just got shredded... but made it up. AP for first 8 min was right at threshold. My power dropped about 40 watts over the next 10 min lol... Another surge went and I was mentally (and possibly physically) unable to go with it, so I just settled in to another 30 min or so of sweet spot. Eventually formed a group of three and lead it out at ~5.0 for the last 3 min so they could duke it out. Had fun.

3x20' last night. Haven't done any focused threshold work in a while, and as such, these sucked. Heart rate was lower than expected for the exertion, did them in ERG mode on the trainer and had to partially lower the target (moving between 260 and 270 depending on how I felt at the moment). Averaged 268, 266, and 266. Hoping it comes back soon and that the (somewhat sporadic) vo2 and higher intensity work lifts my ceiling a little bit.
Also, I'm really digging intervals.icu. I might start using this instead of trainingpeaks, and am considering donating as well.
Barrio Logan P12 this weekend. Payout is 20 deep and so far only 12 registered, let's go!

Typical Tuesday routine, ~10 mi road bike commute and now heading to the track. I think some of the rockstar juniors might be at road nats, and I think there is some pro thing also drawing away a couple other riders, so I may be at the pointy end instead of finding myself in no man's land between the top guys and the pack-fodder Men's A's.
They have a madison on tap for the night as well. Haven't raced one of those since 2019, but then again, no one else has either... They have another 10k scratch race for non-madison riders. Not sure what I'm gonna do, we'll see.

TMonk Did you say have a great ride today right before you turned on to a bike path? We did some efforts on Fiesta and will be at VSC tomorrow for motor work.

Hermes I don't think so, don't remember seeing you guys today. I did ride by the river path on Morena at like 7:30 or so before work, then up the I15 bike path later around 5:30.

Carson velodrome motor paced workout last night. 3x8 lap motor paced 2k pursuits. The first 2k I rode the last lap by myself, the 2nd, 2 laps alone and the 3rd, 3 laps alone.
Here is a video of the last 2k pursuit (8 laps) final two laps (the first lap alone was not recorded) - 31.8 mph, 425 watts, 17.6 second laps. Another chainless day. This is the fastest 3 laps I have recorded at VSC in 11 years. That was a flying 52.8 second 750 meters. I must need two previous pursuits in my legs plus 5 lap lead out and someone following me on a motor screaming faster

Nonstop sweetspot in the windswept lovely 80 deg eastern NC. Felt great. Just motor at 250w or so in TT for an hour. Little under and over with little pimples of rise and fall. The HR per power I like. I like lots.
Later next week little Z2 mini camp with the kiddos out of town for several days. Will take some discipline not to ride too hard or too long at any point. Which would defeat the purpose of the volume.

Track was fun the other night, mostly because there were 2-3 notable hitters that we missing. So in the 30 laps points race (sprints every 6), when a local TT ace (Greg) attacked after sprint number two, I jumped on right away. We got a gap and the field didn't respond right away so we swept up the rest of the points trading half lap pulls. After the next sprint I gave him a whole lap so we could swap the rotation and he could get the next set of points. I really screwed up by not jumping under him on the last lap, thought I would just accelerate out of his draft out of the last corner - mistake. He doesn't have big acceleration and it takes him a while to get going, but a whole lap of winding it up and thinking I could come around - yeah. mistake.
The madison was fun, by half way through I wasn't so spooked by the exchanges. My partner wasn't as fit as me and so after a few exchanges I had to sprint and dig full gas to get back on to the leaders, and after a few of those I told him "smooth, lets TT this". So we lost the front, but it was fun to get back in the groove of doing madison exchanges again.
Yesterday I took the TT bike out for 2x20' at z4 in Fiesta Island, and my hammies were surpassingly supple and forgiving after track the night before. I thought it would be a real struggle.
Off day today, light spin on the race bike tomorrow to make sure things are working, then Barrio Logan P/1/2 on Saturday. It's a tough course with a hill and 8 corners in it, so its gonna be tough. I would consider doubling up and going M35+ 1-4 60 min after the start, but I will have family watching and we are going out to an Italian lunch after. It's a tough course anyway so I might be cooked. Either way I'll be heading down to the course again in the evening to volunteer for course take down. Then ill cap the week off w some volume on Sunday.
