Ever roll a tubular?
Front or back? Glue or tape?
Was it a rush job the night before the race? Or was it the 72 hour three coats of glue in the clean room?

No (never ridden one).
Vaguely on topic, Velo News did a how-to piece awhile back that's pretty methodical in its approach. Hopefully it will take some of the "black magic" guesswork out of the process.
... and since Velo News makes it damn near impossible to just link to the videos in situations like this.

Low speed, around 10 mph, taped tubular on a fresh rim. Rear wheel caught a goofy off camber square edged rock and slipped right off. Not fun. Trashed the rim pretty quick as well. I now run the X-TREME SUPER GONZO tape and haven't had a single problem.

bad glue job on a front rim, I made a sharp turn and peeled the tire off the rim. I fell, then I deflated the tire, put it back on the rime, reinflated and rode home....

Once. Tire was going soft on lap one, lap two I could feel the valve tapping when I hit the pavement section, lap three, I tossed the bike into a hard left on the grass section, and off came the tire. Turned out the valve core was leaking after I aired the tire pre-race. I didn't have a spare bike or wheels and had driven 4 hours ot the race. Hence, why I kept riding until, as it were, the tires came off.
Up to then and since, never had one come off the rim unless I was removing it.

I rolled a front in a practice session. I was skidding out on an off camber and it rolled. I didn't use enough glue. I use glue and tape now and it's next to impossible to remove the tire.

Yep, leading a race by 45 seconds on the last lap. Went to pass a lapped rider and hit a rut on an off camber turn and slammed it hard. Front tire rolled right off. Wasn't a rush job, but I glued it like a road tubular, so not enough glue to hold the tire on at 30psi evidently. Went home and promptly put the tape/glue combo and haven't had the first problem since.
